Instructions for closing your IN Unemployment account:

No action necessary. Teamworks will close your unemployment account.


Instructions for closing your IN Withholding account:
  1. Please fill out the BC-100 form by:
    • Filling out your company information.
    • Checking the Withholding box and indicating the day before your Teamworks start date.
    • Sign and date.
    • Send to the address listed on the form.
  2. As supplemental documentation, include a notarized statement indicating that:
    • [Company] became a PEO client of Teamworks Group Professional Employer (FEIN: 87-0567952 on [Teamworks start date]. Therefore, [Company]’s withholding tax information is being reported under Teamworks withholding tax account effective [Teamworks start date], ongoing.
  3. Fax or mail the form and the notarized statement to the address on the form.
