Utah - Closing Unemployment and Withholding Accounts

Instructions for closing UT Unemployment account:

Teamworks should be able close out your existing unemployment account (if applicable) during the onboarding process. If there seems to be an issue where your account has remained open, please reach out to us.

Instructions for closing UT Withholding account:

  • To close your Utah Withholding account please complete form TC-69c.
  • You'll need to fill in Section 1, 1b, and 1c. Section 3 just needs the closure date (the day before your start date with Teamworks) and signature on the bottom. There is a fax number on the form you can use to send the form to the state.
